Lateral cruras length trimming to deproject the tip

This is a basic surgical maneuver in most rhinoplasties of almost any kind, either primary or revisional, whenever the lateral cruras of the lower lateral cartilages show an excessive length and over support or over project the tip of the nose, needing full thickness resection of a segment, with suture securing of the assembly or letting the lateral crura float free.

It is carried out with scissors resection of the cartilaginous outer end of the lateral cruras with great care not to commit over resection weakening which might potentially lead to collapsed tip, under projection, polly beak deformity, poorly defined tip, etc, in order to prevent deformities requiring revision in the future.

This gesture can be done safely by either closed or open approach rhinoplasties.
