This is a basic surgical maneuver in most rhinoplasties of almost any kind, either primary or revisional, whenever the lateral cruras of the lower lateral cartilages show a prominent bump at the tip of the nose or, should it be smooth but raised, needs en bloc lowering or a reduction of the angle formed between the tip and the dorsum.
It is carried out with scissors resection of the cartilaginous cephalic or upper edge of the lateral cruras with great care not to commit over resection weakening which might potentially lead to pinched nose, pinched nasal valve, collapse of the nostril rim, alar rim retraction, etc, in order to prevent deformities requiring revision in the future.
This gesture can be done safely by either closed or open approach rhinoplasties.
