Vertical tarsoconjunctival müllerectomy shortening of the upper eyelid

Initial level in treating the aging upper eyelid ptosis functional opening or elevation impairment of a slight to moderate grade is the shortening of its levator mechanism by en bloc trimming of the tarsal cartilage, the conjunctiva and the Müller muscle in a vertical fashion and subsequent layered closure.

This maneuver tensions up the upper eyelid levator allowing its reinforcement and opening.
