Certain noses have tight ligaments connecting the skin and cartilages of the columella and the tip to facial muscles like the orbicularis oris, depressor septi nasi and other, leading to a typical deformity when smiling and strong facial expressions by pulling down from the tip, creating polly beak deformity and increasing the dorsal hump, all virtually. Transecting the involved muscles during a rhinoplasty may produce relief of this small but sometimes highly concerning issue.
