Certain noses have tight ligaments connecting the skin and cartilages of the alae and nostrils to facial muscles like the levator labii superioris alaeque nasi muscle, nasalis and other, leading to a typical deformity when smiling and strong facial expressions by pulling up and lateral from the alae and the nostrils, creating augmented effect of alar flare and large nostrils, all virtually. Transecting the involved muscles during a rhinoplasty may produce relief of this small but sometimes highly concerning issue.
