Among the most typical complications of augmentation mammoplasty and one of the causes motivating a revision mammoplasty procedure is the excessive movement of the breast implants; reason why is the necessity of reoperating such mobile breast implant, which is sometimes refractory to conservative therapy, this means the mammary prostheses outer scar is not in contact with the implant itself, does not stick to the implant and fixation is poor null leading to easy mobility of the prosthesis within its capsule and over the chest, something unsightly and very much uncomfortable; overtime the looseness of the implant may likely produce a series of secondary deformities like bottoming out, symmastia, tenting, side boob, animation deformity, double bubble, flipping, seroma, visibility, etc , therefore if a persistent excessive movement scenario occurs to the breast implant it will have to be operated to be reattached and the subsidiary deformities corrected sooner or later.
Causes of breast implants excessive movement are varied, including the increased risk with certain models of implants like smooth shell ones, lack of proper, strict and long postoperative immobility from patient's side to allow prosthetic fixation, serum, blood or milk fluid periprosthetic build ups, infection, genetical factors leading to weak scarring, surgical technique, etc; the state of the art is nowadays with areolar incision and macrotextured shell, cohesive silicone gel filled and anatomical shape breast implants placed in subfascial plane, being an upgraded option the use of highly cohesive gel filled breast implants to reduce rippling.
Once the breast implants excessive movement is confirmed and its irreversibility stated, the recommended treatment protocol may involve performing a late and scheduled capsulectomy once the tissues are cooled down, with or without implant renewal to a more adherent one, preferable polyurethane coated anatomical implants to shape the breast and prevent the migration thanks to the bio velcro of the polyurethane shell, adding any surgical procedure to repair the associated deformities.
